Pharmacy Math for the PTCB

Pharmacy math is the part of the PTCB (PTCE) that rewards practice the most, because the same handful of formulas appear again and again. How much math is on the PTCB?

Calculations are not a separate section. They are spread across the exam and cluster in the Order Entry and Processing domain, which is 22.5% of the scored content. You will also meet dosing and conversion math inside the Medications domain. The PTCE includes an on-screen calculator, so raw arithmetic is not the obstacle. The skill being tested is reading a prescription or an order, translating it into a formula, and keeping your units consistent from start to finish.

These are the formulas that carry most of the exam. Learn to recognize which one a question is asking for, and the rest is bookkeeping.

The calculation guides

Each guide states the formula, walks through two to three worked examples, flags the mistake students make most, and ends with a short practice block.

  • Alligation: the tic-tac-toe method for mixing two strengths to reach a target strength, and how to turn parts into quantities.
  • Concentrations and Dilutions: percentage strength (w/v, w/w, v/v), ratio strength, and the C1 × V1 = C2 × V2 dilution equation.
  • IV Flow Rates: flow rate in mL/hr, drip rate in gtt/min using the drop factor, and infusion time.
  • Conversions: metric, household to metric, temperature, and Roman numerals as written on prescriptions.
  • Days Supply: how long a quantity lasts, including the drops, insulin, and inhaler cases that trip people up.

How to study pharmacy math

A few habits separate students who dread these questions from students who look forward to them:

  • Translate the sig into plain English before you touch a number. Most errors happen in the reading, not the math.
  • Write the unit beside every number. When units cancel cleanly, the setup is right.
  • Estimate the answer first. If your calculated value is ten times off, a decimal slipped.
  • Know your rounding rule for each type: days supply rounds down, drip rates round to the nearest whole drop.
  • Drill one calculation type until the setup is automatic, then mix them the way the exam does.

Is a calculator allowed on the PTCB exam?

Yes. The PTCE provides an on-screen calculator, so you do not need to do long division in your head. The exam tests whether you can set the problem up correctly and keep your units consistent, not whether you can multiply quickly.

What pharmacy math formulas should I memorize for the PTCB?

Do I round pharmacy calculations up or down?

It depends on the quantity. Days supply rounds down, because a partial day is not a full day of therapy. Drip rates round to the nearest whole drop. Doses are rounded to what can be measured with the available device. Read each question for the unit it wants.
