What is not tested
You do not need to know which tetracycline is preferred for which organism, or the details of resistance mechanisms. What matters is the stem, the brand and generic pairs, the one-line mechanism, and the safety and counseling profile, which is unusually rich for this class. Strengths, dosage forms, routes, special handling, and duration of therapy fall under PTCE knowledge area 1.4 and are testable.
Why tetracyclines matter
Doxycycline is among the most dispensed antibiotics in the United States and is used for everything from acne to Lyme disease to pneumonia. The class also gives the exam two of its cleanest counseling questions: why a patient must stay upright after a dose, and why a child under 8 should not receive it.

Mechanism of action
Tetracyclines enter the bacterial cell and bind reversibly to the 30S subunit of the ribosome. That binding site sits where the aminoacyl transfer RNA has to dock in order to hand its amino acid to the growing peptide chain, so with the drug in place the chain simply cannot be extended. Protein synthesis halts and the bacterium stops multiplying, which makes the class bacteriostatic rather than bactericidal. Human ribosomes have a 40S small subunit rather than a 30S, and human cells do not concentrate the drug the way bacteria do, which provides the selectivity. The chemical structure that makes tetracyclines work is also what causes their two most distinctive problems. The molecule has multiple oxygen atoms arranged to grip metal ions, so calcium, magnesium, aluminium, iron, and zinc form insoluble chelates with it in the gut that cannot be absorbed. That same affinity for calcium explains the tooth and bone effects: the drug binds calcium in developing teeth and deposits there permanently, staining them, which is why the class is avoided in children under 8 years old and during the second half of pregnancy. Doxycycline and minocycline are absorbed better than older tetracycline and are less affected by food, though dairy and supplements still matter.

- Use sunscreen and cover up outdoors, because sunburn happens faster and more severely during therapy
